Dear Patient,

I write this letter to my patients past and present with a heavy heart. I have experienced some changes in life including my own and my family members health issues. I feel honored to be the one that you chose to be your family physician over the years. As of January 30th, 2023 I will no longer be doing any primary care medicine and instead focusing my efforts on Hospital medicine and my Hormone replacement (Vintage Medical Group) clinic.

I understand that changing to a new doctor is a difficult process for many patients. I have tried to make this a smooth transition by arranging for Dr John Schoonmaker to take over my primary care practice. I personally selected and feel confident that Dr Schoonmaker is a well-trained, empathetic and caring Family Physician that I think will be a great match for my patients.  As an added bonus Dr Schoonmaker is contracted with a large number of insurance/ health plans which should give you better access to affordable care moving forward.

Below is a list of questions and answers that I hope will make this transition a little smoother. I will be available for a limited number of appointments at Dr Schoonmakers office between now and 1/30/23 to assist both my patients and Dr Schoonmaker to try to make this change hassle free and to maintain continuity of care along with refills etc.

What about my chart?

Dr Schoonmaker will be provided electronic copies and become the custodian of my primary practice patients' medical records.

 

What if I want to continue my care with Dr Schoonmaker?

If you wish to continue with Dr Schoonmaker no action is necessary other than to call to schedule an appointment! He is open for appointments now!!

What if I want to change to a different primary Doctor?

If for some reason you wish to select another provider, you will need to complete a medical records request at your new providers office and have it forwarded to Dr Schoonmaker's office.

What if I am a Vintage Medical Group patient?

Keep your appointment as usual. The office location for your appointment has changed to:

What if I am on hormone replacement therapy but not a Vintage Medical Group Patient?

If you wish you can make an appointment and follow up at the Vintage Medical Group clinic as above.

What about medication refills?

Dr Mitzner will be available to assist with refills if necessary while you are getting established with Dr Schoonmaker or other provider.
